Insurance cos are a very small portion of overall healthcare costs. They're actually limited to 20% of the total cost by federal law (ACA).

Except they have tried to crack down on the doctors and hospital admins' billing fraud forever, and every time they do, people side with the people doing the fraud. See last year's debate about them scrutinizing bills from anesthesiologists (avg yearly income: $491k per year). The doctors and hospitals just have very good PR.

And the execs are certainly not taking home millions - these are regulated middlemen with no rare skills who do nothing, so that would be irrational. They have to compete against insurers that don't have those business lines. And people with those other insurers' plans have the same cost issues...the cost issue isn't unique to insurers' combined with providers like UHC.

Otoh yes it’s mainly the parts through the system where we allow providers to charge hundreds or thousands per patient when the exact same service/procedure/drug costs less than 1% as much in every other developed economy. That plus the AMA creating artificial doctor scarcity with its wildly outdated approach to medical education/training and its obvious mandate to maintain high compensation for its existing members.
But the insurance companies are a key driver of the cost escalation and the increase in costs driven by delaying and avoiding treatment because their entire business is built on rug pulling patients and surprising them with catastrophic bullshit charges that are never revealed to the patient until well after treatment occurs. It also encourages the average patient to think like a deadbeat/system cheater and just try to avoid paying and maintain endless stamina to argue and pushback and wait them out for a settlement offer that includes a massive discount. Ie the “smart” way to deal with them is to avoid paying and make them slash their costs on the backend and depress the reimbursement rates, which of course causes providers to raise prices. It’s all utterly retarded.

It’s a huge miss because the revenue is mostly pre-contracted and they guide the analysts so estimates should be approximately right. Also when a big company that needs a good quarter misses by this much it means they pulled out all the stops in an effort to hit the number and couldn’t get there even with adjustments to accruals and other bullshit. Actual results were almost certainly worse than reported which is why people dump the stock.
