Today, I received a lot of information from (of all places) the hospital that had previously treated him. This is not where he experienced the events causing his death.
First, I received a phone call from the assistant to his surgeon (after hours). She told me she heard what happened, that she's extremely sorry for my loss, etc. She also told me the entire staff is shocked. She told me his doctor would be calling me within the next 10-15 minutes, but that she wanted to help me get whatever info I wanted. She literally logged into Epic and went to his charts at the other hospitals, provided timestamps of when procedures occurred, gave me doctors' names and numbers, etc.
Minutes later, I received a call from the surgeon. Her assistant had actually taken great notes and passed them over to the surgeon and we basically went through the list one-by-one.
Yesterday at 13:45, his blood pressure at MGH was 108/78, pulse 63 bpm, O2 98% on room air. He was then transported to Spaulding Hospital in Cambridge.
He arrived at Spaulding at 14:11. He was "chatting away" according to the intake notes. His vitals were normal.
At 15:56, I received a call from my mom--she was bawling.
At 16:01, I received a call from the doctor at Spaulding: Apparently my dad was in cardiac arrest. He noticed a large amount of "coffee ground emesis" occluding his airway. He called 9-1-1 and EMS was able to secure the airway. All the while, CPR was being performed. It was an unwitnessed arrest.
He arrived at Cambridge Alliance Hospital roughly 10 minutes later (it's only 1000 feet away). We were waiting for updates for another three hours. All the while, he had been going in and hour of cardiac arrest. By about 19:30, the doctor called me to explain the grim news. I explained my father's wishes, and then told him that after four rounds of ACLS drugs, an unwitnessed arrest, etc. I would agree that it is now appropriate to discontinue resuscitation efforts.
Maybe 20 minutes later, the doctor called me back to let me know that he had a pulse, but it was weak and thready. I told him that if he keeps that pulse or if it improves, we will resume resuscitation efforts.
At 20:39, he called me to let me know that my dad passed away at 20:37.
Talking to the surgeon today was incredible. She told me that there's a formal review process happening at MGH for this specific incident and that the same thing his happening at Cambridge Health Alliance. She also strongly suggested getting an autopsy and told me that I need to immediately contact the medical examiner. She mentioned that the entire ICU staff is gutted and they brought in grief counselors specifically for this event.
The medical examiner referred me to the Cambridge Police Department and an officer wants to meet with me tomorrow.
