I am afraid the problem with primary care is that it's actually hard work. 15mins/patient. I understand that irl they don't have to do a full physical, they only ask 1 or 2 questions that require thought and pretend to look at your eyes and nose with a head lamp. but they still have to move fast.. and then sometimes you get patients that come back to you after getting their "aids stabilized" or "cancer stabilized". I guess they come to primary care to discuss their "depression and end of life care".

just remembered another usmle question. a fat woman comes you and says "I am sure it's my thyroid" but her thyroid level is normal. so what should you say to her? I forgot what I put there.

but what is easy? at least with primary care I could do a residency in California and it's only 3yrs and only 40hrs/wk. general surgery would have to go somewhere like Midwest ttt. and that's at least 5yrs of 80hrs/wk. is anesthesia easy? I doubt.

I would think Alzheimer is hard and doesn't pay.. because u actually have to discuss those 'quality of life' issues and endoflife care. on sdn it says that neuro no longer makes any $ on emg so it makes outpatient unprofitable. in that case they have to do inpatient, i.e. stroke. but I think there is less demand for stroke than for regular IM hospitalists and pay is the same. and I don't think it's fun to examine a stroke patient for speech and comprehension problems if you yourself have an accent.
