A common reptilian complaint directed against Obamacare is its irresponsible in cost-management.

The last year of life is where costs go nuts and the quality of life is awful. It's time for a sober discussion about the necessity of extreme care in the old. Our politics are too demagogued with rhetoric about death panels and euthanasia though.

stop paying doctors and hospitals so much damn money for so little work. remember that there are medicare rates for every single procedure. Just slash these by 30-40%.

Greetings,

PCPs make WAY too much money for very little work. I went to a new doc w/o my insurance card one time, so I had to submit the final bill to my insurance provider later. So I got the actual "uninsured dood" bill.

Bill was over $100 for a 15 minute meeting where the doc LITERALLY just wrote me a three-month refill on blood pressure medication. Not even 15minutes with doc. 5 mins of getting weighed by a nurse, having bp taken, etc; 5 minutes of sitting in cold room alone; 5 minutes of doctor saying 'hi bye here's your script'

Reminder: PCPs are, on average, REALLY FUCKING DUMB. Like barely made it into med school, barely passed classes, couldn't get even a remotely decent residency, etc.

It's like a Cooley Law grad billing $400/hr to spellcheck your pro se petition.

Ding. PCPs are generally the ones who work the most for the least pay. They have to deal with the most people. Very few people pay the "uninsured" rates. Chances are w/ insurance that would have cost like 20 bucks.

Specialist/surgeon rates are absurd, even w/ insurance.

vis a vis specialty providers, pcps tend to accommodate higher patient volumes, yes. with this, tho, pcps are making comparatively superficial diagnoses.

End fantasy of dollar one coverage.

Encourage catastrophic policies.

Introduced sensible market based reforms that introduce competition (e.g. end absurd prohibitions on selling insurance across state lines)

nix fats

nix malpractice lawyers

nix rent seeking medical professionals

nix new innovative practices that cost 10x more than old ones but are only 1% better

nix intensive end of life care, if 100,000 isn't going to save you, you're done unless you can pay out of pocket

nix health insurance if you swallow more than some amount of fat per year or if you exercise less than 4 days a week

how about basic universal coverage for everyone with high deductibles

if you want extra high tech shit you have to pay out of pocket

if you want life extending type care after age 85 you have to pay out of pocket

open more med schools; lower standards; train more nurses; negotiate for lower prescription drug prices; make med mal best practices safe harbors; disincentive docs from elective practices somehow

Stop subsidizing medical services.

Stop regulating nurses out of jobs to protect doctors.

Stop subsidizing medical schools.

Require public posting of rates for services.

Create a national single payer system that covers the first $1,000 and everything over $20,000 per year. Then let people either pay for the $19,000 (if needed) or get an additional private insurance to pay for it.

The desired effect would be that health care consumers would actually know how much their care costs and would be more selective. Hopefully this would encourage competition and innovation, while not subjecting everyone with a major medical problem to bankruptcy.
