He said that something was missed in the maintenance cycle and those pilots never had a chance. He theorized that their was a wing tank fuel leak that leaked into the engine, the engine overheated/overrevved and spit out debris that killed the tail engine, both engines stopped working, and plane cant fly on third engine alone especially when super heavy at takeoff.
