How Being a Doctor Became the Most Miserable Profession

Nine of 10 doctors discourage others from joining the profession, and 300 physicians commit suicide every year. When did it get this bad?

Simply put, being a doctor has become a miserable and humiliating undertaking. Indeed, many doctors feel that America has declared war on physicians—and both physicians and patients are the losers.

Not surprisingly, many doctors want out. Medical students opt for high-paying specialties so they can retire as quickly as possible. Physician MBA programs—that promise doctors a way into management—are flourishing. The website known as the Drop-Out-Club—which hooks doctors up with jobs at hedge funds and venture capital firms—has a solid following. In fact, physicians are so bummed out that 9 out of 10 doctors would discourage anyone from entering the profession.

Neither patients nor doctors are happy about [doctors spending such a significant amount of time processing insurance claims]. What worries many doctors, however, is that the Affordable Care Act has codified this broken system into law. While forcing everyone to buy health insurance, ACA might have mandated a uniform or streamlined claims procedure that would have gone a long way to improving access to care. As Malcolm Gladwell noted, “You don’t train someone for all of those years in [medicine]… and then have them run a claims processing operation for insurance companies.”

In fact, difficulty dealing with insurers has caused many physicians to close their practices and become employees. But for patients, seeing an employed doctor doesn’t give them more time with the doctor—since employed physicians also have high patient loads. “A panel size of 2,000 to 2,500 patients is too many,” says Dr. Schimpff. That’s the number of patients primary care doctors typically are forced to carry—and that means seeing 24 or more patients a day, and often these patients have 10 or more medical problems. As any seasoned physician knows, this is do-able, but it’s certainly not optimal.

complete bullshit...

doctors are mad because they "only" make 300k while their peers in tech & finance make much more

what they don't realize is that those elite tech/finance gigs are very limited and only exist in NYC/SF

but most doctors are striver fucks w/ god complexes who think they deserve to make more than anyone, so the thought of non-doctors making more makes them little crybabies who want to quit

lol yeah okay a couple years of working. unless they want to live like poors during their 30s, try more like 10. so they will be like 45+ when they finally have a positive net worth. not saying that going into med is a bad investment, but golden handcuffs are real and take a psychological toll.

plastics & derm are the tippy top of the mark, but there are so many high-paying specalities, surgical specialities, etc. I know some dumb chick who did a gastro fellowship, another average dood doing an ortho fellowship.

of course, they are not dumb, probably median intelligence amongst their peers, but they will be doing great

gastro is the MOST competitive fellowship out of IM and ortho residencies are only ~800 out of the 30k. the VAST majority are primary care. why do you think specialist pay is so much higher? restricted supply. if there were a ton of specialists, then they sure as hell wouldn't be pulling in 500k like they are now.

um, no.

Also, many specialists don't even make that much. Loads of endocrinologists, nephrologists, pulmonary/CC docs make less than or slightly above FM or hospitalists.

OB/GYN gets slaughtered by malpractice, so many docs go to practiing strictly GYN which isn't very profitable.

Cards, GI, Heme/Onc are the real high paying medicine specialties, and those fellowships are very hard to match into, especially from a general community-type residency.

This does not comport with my experience in the least. The doctors I've known -- even, or perhaps, especially, the ones who are considered to be "top" in their profession (e.g., the surgical mastermen) -- tend to be quiet, humble, and unassuming, quite contrary to the image of an "alpha" doctor that you'll see from time to time on television. Think more multi-millionaire-next-door types than ... I don't know, Alpha George Clooney racing through the Canyons in his Ferrari after an emergency surgery, model on his side.

Having said that, most of the physicians I know are established -- I don't know many physicians just starting out and maybe there's been a generational shift in attitude with the latest crop of bright-eyed healers. Two shots to Tut; and a snitch named Haitian Jack; this is the realest shit I ever wrote; the truest shit I ever spoke.

I wouldn't let people anonymously bitching on boggleheads characterize the profession. That said, I think it is fair to say that many established physicians -- including the successful ones -- do go around telling people not to enter the profession any longer, mostly out of a sense that things are going to change drastically over the next couple decades. But that doesn't mean that they have these caricatured aggressive, craven personalities as suggested above.

the kind of doctor who grinds throughout college, studies nonstop every day of the week during medical school, and works 100 hour weeks every week during their 7 year residency making sub-minimum wage probably has the competency to land an IBD/tech gig so no doubt they're MAF about the opportunity cost of their profession

they should just try to be like this dood and do some finance on the side http://en.wikipedia.org/wiki/Michael_Burry

To be fair,

XO on being a doctor: "Oh boo hoo, cry me a fucking river--you only make $200k a year? How ever will you service your six figures of debt and quickly pay it all off, all while living a nice upper middle class lifestyle in your 30s and enjoying a considerable degree of social prestige? Too bad these whiny fucks think they are are better than everyone else and deserve to be paid the most out of any profession because they are 'uniquely miserable' or something. And lol @ all of them thinking they could have made it in finance."

XO on being a biglawyer: "LJL @ this shit profession that works me to the bone and keeps me checking my BB and canceling night and weekend plans, all to do a bunch of boring paper pushing. Maybe if I was getting rich it would be worth it, but I have six figures worth of debt to pay down so I feel broke as fuck in addition to being constantly stressed. I should have just done entrepreneurship and paid myself. Law is the worst. Fuck this shit."

i know several doctors and they all love it now that theyre done with their fellowships.

one clears 900k a year with his geriatric practice in suburban phoenix.

another is married to a doctor and they have 4 young kids together and basically both work half-time for one full-time doctors salary.

one "only" makes 150k in a third-tier city working 40 hours a week and taking call one week a month

You expect us to believe that a new doctor in a low-paying specialty makes an extremely high income?

well you dont know shit about how doctors make money then brother. if you have the patients (and the sun city/sunc city west area of phoenix has lots of geriatric folks) then you can bill your ass off using PAs and LPNs. I'm sure the doc works very long hours but he's obviously in it for the money

Almost every private practice doctor has nurses and whatnot. You're asking us to believe that a young doctor in a low-paying specialty is making an extremely unusual income (that you just happen to know).

Sure, it happens. Lottery winners exist. But I still don't believe you.

not every doctor even owns his own practice. this guy wanted to gun hard, and 3 years after a geriatric residency this is what he makes with about 5 PAs and some other staff. whether you believe me or not is kind of a matter of indifference to me at this point. I'm not a doctor, I'm not bragging about myself, I'm just letting you know that as a doctor you can either (1) ball, or (2) live a chill lifestyle if you know how to play the game.

I promise you these facts were conveyed to me by the guy himself. He was certainly bragging, and possibly exaggerating, but this is what he said and how he explained it.

not to outself, but I'm related to a person that teaches an internal medicine residency. these are former residents.

flame that he had the capital to start a practice right after residency. did he not have student loans?

reimbursement for geriatrics is so low that it barely covers overhead for most practices, so for him to move into a market and be able to amass the kind of patient volume needed to generate 900k personal profit for him (think of how much he'd need to gross to cop this) is ludicrous

Do you? Or are you just lapping up whatever this "buddy" tells you? Geriatrics is basically regular internal medicine, just for old people. It's not a very high paying specialty. Could someone net $900k doing this? Yeah, I'm sure it's possible, but they would be in the top .001% of their profession. If it's true then more power to your bro.

This is what makes me doubt it even more, since places like Arizona and S. Fla are over-saturated with doctors and medical professionals catering to retirees. The competition in these areas is fierce, which actually keeps earning potential low.

I don't have hard numbers in front of me, and I could be wrong, but yes, in general, major metro markets tend to have an oversupply of doctors relative to demand, which is why when you look at regional salary surveys for MDs, the highest salaries tend to be in the Mountain West and rural South East, and not in places like CA, NY and FL

note that i said in NYC. it's probably around 200k. now pull up some numbers on geriatric reimbursement in a private practice setting. these guys don't do the big procedures that rake in money. his practice is entirely volume based, so for him to amass the kind of volume out of residency to clear 900k after the overhead of a 15 employee practice is insane. if it happened, good for him, but trying to say this is typical or within reach for the average IM doc leaving residency with 250k debt is disingenuous

MDs who go to school on the military's dime owe 7 years on active duty, and that does not count your time in medical school, internship or residency.

This is an elaborate flame created by the AMA to keep demand higher than supply

To be fair, if you have a 30 MCAT, you're getting into at least a DO school. You wanna open the floodgates and let shithea CVds in?

If you have a 30 MCAT, you might be guaranteed admission to Rocky Vista or some other festering TTT, but that's about it. I'm not sure if even Rocky Vista would take a 30 MCAT with a shit GPA and no shadowing/volunteering/research. I know you probably know this, but I get annoyed by the dumb lawyers who think that medical schools admissions is a numbers game like law school admissions. I've known plenty of people with high MCAT scores who got rejected to most (or all) schools where they applied because they didn't have their extracurriculars or they were really aspie during interviews (or both).

best friend is a partner in a private medical practice that makes most of its money leasing space and expensive medical equipment to hospitals. His units gross him in the ballpark of 500k a year. He doesn't hate being a doctor, but he definitely went in thinking it was one thing and realized that it's basically another office job. It's sad in that dude really wants to help people but got caught up in a golden handcuffs deal and supports his WHOLE family. Mom, brothers, aunts in the caribbean, wife's family in the caribbean...the whole nine.
