Nine of 10 doctors discourage others from joining the profession, and 300 physicians commit suicide every year. When did it get this bad?

Simply put, being a doctor has become a miserable and humiliating undertaking. Indeed, many doctors feel that America has declared war on physicians—and both physicians and patients are the losers.

Not surprisingly, many doctors want out. Medical students opt for high-paying specialties so they can retire as quickly as possible. Physician MBA programs—that promise doctors a way into management—are flourishing. The website known as the Drop-Out-Club—which hooks doctors up with jobs at hedge funds and venture capital firms—has a solid following. In fact, physicians are so bummed out that 9 out of 10 doctors would discourage anyone from entering the profession.

Neither patients nor doctors are happy about [doctors spending such a significant amount of time processing insurance claims]. What worries many doctors, however, is that the Affordable Care Act has codified this broken system into law. While forcing everyone to buy health insurance, ACA might have mandated a uniform or streamlined claims procedure that would have gone a long way to improving access to care. As Malcolm Gladwell noted, “You don’t train someone for all of those years in [medicine]… and then have them run a claims processing operation for insurance companies.”

In fact, difficulty dealing with insurers has caused many physicians to close their practices and become employees. But for patients, seeing an employed doctor doesn’t give them more time with the doctor—since employed physicians also have high patient loads. “A panel size of 2,000 to 2,500 patients is too many,” says Dr. Schimpff. That’s the number of patients primary care doctors typically are forced to carry—and that means seeing 24 or more patients a day, and often these patients have 10 or more medical problems. As any seasoned physician knows, this is do-able, but it’s certainly not optimal.

Also, many specialists don't even make that much. Loads of endocrinologists, nephrologists, pulmonary/CC docs make less than or slightly above FM or hospitalists.

OB/GYN gets slaughtered by malpractice, so many docs go to practiing strictly GYN which isn't very profitable.

Cards, GI, Heme/Onc are the real high paying medicine specialties, and those fellowships are very hard to match into, especially from a general community-type residency.

This does not comport with my experience in the least. The doctors I've known -- even, or perhaps, especially, the ones who are considered to be "top" in their profession (e.g., the surgical mastermen) -- tend to be quiet, humble, and unassuming, quite contrary to the image of an "alpha" doctor that you'll see from time to time on television. Think more multi-millionaire-next-door types than ... I don't know, Alpha George Clooney racing through the Canyons in his Ferrari after an emergency surgery, model on his side.

Having said that, most of the physicians I know are established -- I don't know many physicians just starting out and maybe there's been a generational shift in attitude with the latest crop of bright-eyed healers. Two shots to Tut; and a snitch named Haitian Jack; this is the realest shit I ever wrote; the truest shit I ever spoke.

That said, I think it is fair to say that many established physicians -- including the successful ones -- do go around telling people not to enter the profession any longer, mostly out of a sense that things are going to change drastically over the next couple decades. But that doesn't mean that they have these caricatured aggressive, craven personalities as suggested above.

not every doctor even owns his own practice. this guy wanted to gun hard, and 3 years after a geriatric residency this is what he makes with about 5 PAs and some other staff. whether you believe me or not is kind of a matter of indifference to me at this point. I'm not a doctor, I'm not bragging about myself, I'm just letting you know that as a doctor you can either (1) ball, or (2) live a chill lifestyle if you know how to play the game.

I promise you these facts were conveyed to me by the guy himself. He was certainly bragging, and possibly exaggerating, but this is what he said and how he explained it.

I don't have hard numbers in front of me, and I could be wrong, but yes, in general, major metro markets tend to have an oversupply of doctors relative to demand, which is why when you look at regional salary surveys for MDs, the highest salaries tend to be in the Mountain West and rural South East, and not in places like CA, NY and FL

note that i said in NYC. it's probably around 200k. now pull up some numbers on geriatric reimbursement in a private practice setting. these guys don't do the big procedures that rake in money. his practice is entirely volume based, so for him to amass the kind of volume out of residency to clear 900k after the overhead of a 15 employee practice is insane. if it happened, good for him, but trying to say this is typical or within reach for the average IM doc leaving residency with 250k debt is disingenuous

If you have a 30 MCAT, you might be guaranteed admission to Rocky Vista or some other festering TTT, but that's about it. I'm not sure if even Rocky Vista would take a 30 MCAT with a shit GPA and no shadowing/volunteering/research. I know you probably know this, but I get annoyed by the dumb lawyers who think that medical schools admissions is a numbers game like law school admissions. I've known plenty of people with high MCAT scores who got rejected to most (or all) schools where they applied because they didn't have their extracurriculars or they were really aspie during interviews (or both).
