newly minted MDs outside of surgery will be out of a job soon enough. medicare, medicaid, and obamacare subsidies aren't sustainable in the long run unless we cut costs. they'll never touch hospital admins, pharma companies, medical device companies, etc. doctors are an easy target -- factionalized, easy to demonize, and non-union. replace most of them with NPs and have a few MDs supervising and you can cut costs.

of course, this will barely move the needle since physician comp is 6% of healthcare spending, but i'm convinced that's how it'll go. visited my uncle who was hospitalized with cancer (stable condition) and he was only visited by a "DNP" the whole day (doctor of nurse practioning). he called her a doctor because he assumed she was one -- and the dumb bitch never corrected him.

it depends on your specialty and where you are willing to work. most medical fields have a lot of stress because of the procedural nature of the work (cardiology, ENT, anesthesia, to a lesser extent GI and radiology). something like endo or rheum is probably low stress but pays shit like primary care. there's nothing wrong with topping out at mid 100's low 200's per se, but you leave med school with half a million in debt. tl;dr dont do primary care unless someone else is paying.

a lot of fields are also saturated in desirable markets, which diminishes teh negotiating power of the doctor as far as how many patients they have to churn out in a day.
