After figuring in premiums, deductibles, and copays, paying cash for surgery isn't such a dumb idea after all, is it?
When I'm back in the US each summer, I get a full annual dental x-ray and cleaning (no cavities this year) and the nice desk lady reduced my bill from $200 to $150 due to me being a repeat customer. Compare that to what you're paying for insurance monthly all year. Almost every dental plan out there caps your annual benefits at $1500, by the way.
My tonsillectomy six years ago was like $1300 all in - including the ENT practice's pre-surgery and post-surgery visits and the bill for the surgery center and I wasn't billed by eighteen different companies, either. I wasn't paying insurance premiums all year.

I recently let a partial rotator tear heal on its own, never saw anybody, basically 100% on my right shoulder, 90% on my left, would have cost like 1k probably to see someone & get all of the exams, etc., then the doctor who have said some bS about potentially getting surgery, but better to wait it out, or w/ who knows.
its a huge scam. Even as a kid its a huge scam. Just like most of society - but people who are duped are the best participants and get the most rewards. Ignorance is bliss.

2010 - startup $60/mo Aetna $5k deductible
2012 - Wall Street firm $0 $3k in-network/$5k out of network United bullshit
Obamacare passes now I'm paying $188 out of the same firm.
2017 - $450/mo $7500 deductible using Oscar actually better
2018 - remains to be seen
