lol dude I am exactly the same way. It got so bad that I forced myself to do something about it. Here's what I recommend:
1. Devise a strict daily routine, write it down, use some kind of task or calendar app with alerts and stick to this routine RELIGIOUSLY
2. Make sure that meditation or exercise or some kind of activity that combats anxiety is included in the routine
3. Limit your caffeine and coffee intake to the morning, DO NOT drink throughout the day, this will seriously fuck with you, trust me I know
4. Read up on how to do something you've always wanted to do, watch tutorial vids or whatever, and try to really focus on getting it done
5. The more you touch your dick the more you will return to it. It's hard but once you stop wanking all the time just because it's the only thing that feels good you will get over the hump and on your way to rehabilitation.
