Going beyond the recommended dose is bad for you heart because testosterone generally is associated with higher rates of heart disease -- but "cardiotoxic" is the wrong term.
Also, overall physiological improvements from TRT appear to outweigh the negative effects on the heart. That said, you should be sure to have a very good diet if you're on TRT. Also be sure to get the higher resolution cholesterol tests than can differentiate between the different types of LDL.

some preliminary questions:
is this getting it from the black market, or from a prescription? i'm pretty sure i don't have actual low T
is it an injection with a needle or are there other delivery mechanisms?

Go to LifeExtenion.com, buy a test and go to a local labcorp. Get the basic male hormone panel. If you're concerned about hair and/or take finasteride, be sure to also get your DHT tested.
If you're legit in the middle or upper half of the normal range, don't bother with TRT.
If you're in the bottom of or below the range, give it a shot.
I've you're in the normal range, when you go to the doc for testing, go out drinking the night before and then do a heavy deadlift+Squat workout that morning. You will show up on the test with the Ttest levels of a 5 year old.

If you are in solid middle or upper middle range, then I think it's a toss up whether it's worth trying out. You will need a higher dose to get anything out of it, and that has additional risks.
UG lab test sucks. It's loaded with alcohol to keep it safe and that will cause lots of pain and immune system response. And it still will likely give you an infection eventually.

I'm recommending TRT if he meets the clinical criteria.
Also -- it's coming off gear, and using various more androgenic compounds that may lead to emotional instability.
TRT users become much more emotionally stable.
