I've noticed over the years that the prestigious docs have a chip on their shoulders if they're seeing patients every day and demand that they make serious $$$ otherwise. They usually have a machine set up where they see me for like 30 seconds while I'm seen by others before that. These guys must be mad that some members of their med school class are raking in the biotech bux or something like that.
IME The DOs, NP/PAs have all been better at patient care.
