Date: April 8th, 2020 1:34 PM Author: Razzle-dazzle honey-headed hairy legs I don't have any data but all I know is that we've been giving hydroxychloroquine to just about everyone who comes to the ED with presumed covid so it's hard to assess how effective it's been. Let's say this bug has a 1% CFR. If we're looking at mortality as an endpoint, then since 99% will recover without the drug, no single doctor can anecdotally say it "works" without a control group and a sufficient sample size. And for what it's worth, I've seen relatively healthy people in their 50s die with the drug on board since admission, so it's no panacea. I think it may be more fruitful to change the outcome of interest to ICU admissions. If we can keep certain high risk people out of the ICU with the drug, it would do a lot to reduce strain on resources and improve mortality indirectly |

I'm thinking the prophylaxis or very early use possibilities may be more promising, but I'm getting a little concerned that even now it's so unclear. Date: April 8th, 2020 2:03 PM Author: Razzle-dazzle honey-headed hairy legs can you link me to what this article is talking about? on its face, this doesn't make any sense. malaria is a protozoa (not bacteria) that parasitizes red blood cells, digests hemoglobin, and causes the cells to burst, thus lowering oxygen carrying capacity. when the protozoa digest hemoglobin, they create toxic heme byproducts which they actively crystallize in order to neutralize them. hydroxychloroquine and other quinolines work by being taken up in the parasite food vacuoles and prevent the crystallization of the toxic byproducts, so the parasite dies due to the byproducts of its own digestion. this has almost nothing to do with how hydroxychloroquine works for rheumatologic disease, which is immunomodulation, and i assumed something similar was at work here. but given that it has direct in vitro activity against the virus, maybe there is something more to its action. but i just can't see how hemoglobin would be involved because this virus does not infect RBCs and the body's ensuing inflammatory response does not involve RBCs.
edit: looked up HCQ's MOA in covid: "Prevents acidification of endosomes interrupting cellular functions and replication; Prevents viral entry via ACE2 binding; Reduction of viral infectivity; Immunomodulator" |
