InHouse bro here.

I did this to outside counsel a few months ago. I was trying to figure out the timeline of a variety of agreements/amendments, and one K popped up with the name of a foreign affiliate with foreign law scattered throughout the doc (this was exclusively a domestic deal). Hit up outside counsel that prepared the docs to reconcile the discrepancy. Set up a call with my boss, partner, and associate that prepared the doc.

Call was no more than 10 mins, 5 of which was the partner apologizing for not reviewing the associate's work/mix up. Partner said that they'd do the work and write off the time to prepare a new agreement. Turns out the associate/partner fucked up and forgot to update the agreements to reflect the exclusion of our foreign affiliate. Don't know if the associate jumped or was tossed on the grenade. At the time the associate was a 1st year, so I can't imagine he was to blame.

I fucked up pretty bad once. I had a national client in a slew of multi jurisdictional case. I was given rogs to complete and incorrectly identified my clients address do to 90 different ones given and they got subjected to jurisdiction and a shit venue they spent hours if not months determining and coordinating with all counsel to avoid.

I think litigation the mistakes catch you real quick. When you have opposing counsel they usually press issues and your screw ups end up as a motion for sanctions/protective order/compel etc within months.

This doesn't fly for me since I tend to get put in charge of various (usually failing) projects. So I get a "TMF, do you remember that thing you worked on 3 years ago when you just got here? Can you remember why you chose to do A instead of B?" It usually is because of something that was totally different 3 years ago that would have made B impossible but when you're on the spot you usually respond with a horrified, blank look as you desperately try to recall what was going on.
