Cholesterol numbers were high six months ago. Getting another test tomorrow and I assume numbers will still be high.

The bad:

I'm around 20 pounds overweight. Stress from work made me gain the weight, and I can't lose it.

The good:

I exercise for at least two hours every single day. Some combination of brisk walking, stationary bike, and weights.

Do I need to get on a statin?

that is borderline bad. there is a 16x higher risk of heart disease if your triglyceride to HDL ratio is 3.5 or above. yours is 2.82, so not quite there, but borderline.

HDL is increased via exercise, i've had some success lowering my triglycerides by cutting out coffee (in a subset of individuals triglycerides are spiked by coffee use, but not in everyone). you can google how to cut triglycerides, people have various recommendations

Totally overly broad. The literature is pretty favorable to non-butter dairy. Hard cheese and unsweetened yogurt don’t appear to raise bad cholesterol. These feature heavily in diets like DASH and Mediterranean, which promote heart health. Best theory is that the milk fat globule remains unrefined in foods like cheese and yogurt so the liver somehow isn’t as troubled with the saturated fat in those foods. The milk fat globule in butter, however, gets whipped up in the refining process and becomes atherogenic. So your advice to ditch all dairy is retarded. That being said, OP should track his diet and lipid numbers in case his body reacts idiosyncratically to whole fat dairy.

Meat is a different story. Basically all meat fat raises LDL, but there’s a wide range. Sticking with fish is generally best (omega-3 is cardio protective in other ways). Lean game meat is probably fine. As is chicken breast.

Major lipidology CSVD-prevention MFE here.

1) Get your APO-B tested and focus on that moving forward instead of LDL-C. LDL-C is just a proxy for APO-B. You can order for yourself from laborp (via LifeExtension.com) for just $20. Don't have time to explain why now, but the best current science says that you can totally ignore HDL. Same goes for HDL/LDL ratio. Anyone paying attention to that is operating on outdated science. That said, if you want to pay attention to HDL -- get your APO-A measured instead. You can get a cheap APO-A/B ratio test that will give you both.

2) Get your LP(a) tested. If it's not high, you don't need to get it measured again.

3) Cut out all sugar, processed foods, high GI / GL foods.

4) Limit your saturated fat intake from animal sources.

5) Eat a shit load of veggies and fiber. Eat fermented foods every day.

6) Eat a shit load of organic tree nuts each day.

8) Up your excercise even more

9) Get your APO-B tested again -- twice -- 3-4 days apart. If still high then consider medication. If your insurance will cover it, or if you're loaded, try a PCSK9 inhibitor like Repatha first. If not, first try Ezetamibe and Bempadoic acid. Stack on a low-dose statin if still high after that.

10) If you still can't get your APO-B lower and/or don't want to take all those meds, you need to REALLY MAKE SURE that you don't have high blood pressure. If necessary, take blood pressure meds.

11) EXCERCISE, EXCERCISE EXCERCISE.

I would add that non-HDL cholesterol is probably a decent enough marker without having to get an exotic apoB test. I think the non-HDL cholesterol number and apoB are rarely discordant from what I recall. You just take your total cholesterol number and subtract HDL.

Lowering the amount in your lipoproteins (or more importantly, the number of lipoproteins) does not lower the amount of cholesterol available to the cells in your testes to convert to sex steroids

You can absolutely lose the weight. Eat less (and more lean protein), exercise more frequently and and with greater intensity. "Brisk walking" is not gonna fucking cut it. Start doing sprint intervals on the bike, using an assault bike if your gym has one, jumping rope, swimming, and running. 30 minutes of zone 2 or 3 cardio every day.

For lifting, get on a DUP powerbuilding split. I can provide one if needed.
